"I support patients from GP surgeries with a wide range of needs. Whether it’s individuals struggling to manage at home, dealing with loneliness, requiring equipment to keep them safe or facing various other challenges. My role involves coordinating their care.

Poppy Watkins, a Care Coordinator for Halesowen PCN uses the NHS and Care Volunteer Responders programme’s Community Response service to support those facing health challenges, limited mobility, or simply those struggling to shop unaided. Volunteers collect and deliver food shopping and essential items, helping to keep those being supported safe and well.
