Making a referral for someone else
Friends and family
This guide shows you how to fill out the referral form when requesting support on behalf of someone else.
You must have permission of the person being referred to make a referral on their behalf as you will need to share their details on the form.

Referral details
Select that the support is for someone else, it’s for me is a self-referral.
Then select No in the ‘Are you referring on behalf of an organisation’ drop down menu, this is for professional referrers like GPs.
Then you will need to select the local authority area of the person you are referring, this is so that the support available in that area will display for you to select from.

When you select the type of support the available activities for this category will be displayed. Select the activity you would like to refer the person for. Details of the available activities are in the guide below.

Telephone Support
Check In and Chat Call
You will need to fill out the following details:
- Frequency - One-off, daily, weekly
- If one-off also select the Preferred day/date – yes or no and if yes, the preferred day/date – date and time, followed by the number of weeks the support is required for from 1 to 18 weeks.
- If daily select which day or days Monday to Sunday, followed by the number of weeks the support is required for from 1 to 18 weeks.
- If weekly also select the Preferred day/date – yes or no and if yes, the preferred day/date – date and time, followed by the number of weeks the support is required from 1 to 18 weeks.

Companionship Calls
You will need to fill out the following details:
- Preferred day/date - yes or no
- If yes preferred day/date – select date by clicking on calendar and the time can be selected from the list of available times.

Your details
Enter your details, including your relationship to the person you are referring, this is so you can be contacted if there are any issues or we need to follow up.

Client details
Fill out the details of the person you are referring, the volunteer will need these to make contact so please ensure that they are correct before you submit the form.
You will need to enter the preferred language of the person you are referring, this is so we can try and match a volunteer with this language. You can also request the information letter in a format that suits the person please select from the options available in the drop-down menus.

GP details
Then you need to enter the GP details for the person you are referring. This is so we can contact them if there is a concern such as the volunteer has been unable to make contact.

Declarations
Finally complete the consent box that confirms the person is aware of the referral and that you have their permission and the consent box that confirms they are suitable as per the programme criteria, then select Register Referral.
